Application for Vietnam Entry and Exit 2020 is a form issued by the Vietnam Immigration Department, is required of all foreign passengers arriving in Vietnam by air.
What is entry and exit Vietnam form?
“Entry and exit form” requires your information about your entry and exit Vietnam. You will be handed out the 2 pages of this form either on the airplane or when you arrive at a Vietnam airport.
In case you approved to have Vietnam visa at Vietnam international airport through our Vietnam visa on arrival service, you will also receive “entry and exit form”.
After filling in the form with your information, please return it to the immigration officer who give you a stamp on your passport.
However, you can download the form and fill it in advance to save your time at arrival airport as well.
As normal, the “Entry and Exit Vietnam form” is done on checking in Vietnam airport. However, in the purpose of saving your time at Vietnam international airport, we would like to send this form to customer in advanced to fill out and glue a passport-sized photo on it.
With the photo on this form, you should attach 1 photo which could be in 3×4 cm, 4×6 cm, 5×5 cm but it must have white background.
You can easily see on the bottom of the ‘Application for Entry and Exit Vietnam’ form that you should “Please, fill in one form (attaching one latest photo, in 4 x 6 cm size, with a face straight forward) submit one form to the Vietnamese Immigration Checking point at Vietnamese airport.”
This states that you just need to submit an “Entry and exit Vietnam form” together with a visa approval letter, passport, 1 photo and stamping fee at the Immigration Counter at the airport, don’t need to submit to any other Embassy or Consulate.
Important notices when filling in “Entry and exit form”
As this form is not difficult to fill in, we just talk about some items which may draw special attention of applicants. Here is some guidance that you should take notice when you are about to fill these items:
Item No 6. Kind of passport: you should fill into the “entry and exit form” the kind of passport you are holding: diplomatic passport, official passport or popular passport.

Item No 8. Present address and phone number: this is the place for your permanent address and phone number that will be the easiest for us to contact.

Item No 10. Name, address of office, organization or full name of relation, address of your relatives to contact while in Vietnam: You can ignore this item and don’t need to fill in, just leave it a blank. If you travel with a travel agency, you can note the name of this travel agency in this case.

Item No 12. Entry and exit time: You can fill either status “single” (in case you enter and exit Vietnam one time based on the validity period noted in your visa) or “multiple” (in case you enter and exit Vietnam more than one time based on the validity period noted in your visa).

Item No 14. Done at & on: For “Done at”, you can fill your port of arrival as you are supposed to submit the form and get a stamp on your visa there. For “On”, you should fill in with your real arrival date (this is the day that you check – in at Vietnam airport).

At the footer of the form, for anyone who has a Vietnam visa approval letter for visa on arrival, you should ignore this item when you are going to have your visa stamped at Vietnam international airport and need not to send anything to Vietnam Embassy.
